UnitedHealth's adjusted earnings were $4.69 a share against $2.92 a year earlier, helped by $610 million of favourable reserve development; revenue was $112.7 billion, slightly lower than last year, and UnitedHealthcare served 48.3 million people against 50.1 million.
UnitedHealth Group, the largest health insurer in the United States, reported third-quarter 2026 results on October 13 that show a sharp recovery in profit and a smaller business. Adjusted diluted earnings were $4.69 per share, compared with $2.92 a year earlier, and the company raised its outlook for the full year. This article is based on UnitedHealth's earnings release, filed with the Securities and Exchange Commission, and on a news report of the results from Coin Alert News, a site that mainly covers cryptocurrency, so we use the latter only for market details that the release does not contain and label them. All amounts are in US dollars.
Start with the headline figures from the release. Revenue was $112.7 billion, slightly lower than the $113.2 billion of the third quarter of 2025. Earnings from operations were $6.6 billion, up from $4.3 billion, and net earnings were $4.3 billion, up from $2.5 billion, which is a net margin of 3.6% against 2.1%. Diluted earnings per share were $4.56 on a GAAP basis, against $2.59, and $4.69 adjusted, against $2.92. The news report says the adjusted figure beat a consensus of $4.14 and that revenue was above an estimate of $111.40 billion. Those consensus numbers are from the news site and we have not verified them.
The medical care ratio, the share of premiums paid out in medical costs, improved to 88.4% from 89.9%. That includes $610 million of favourable development of prior-period reserves related to 2026 dates of service, meaning the company's earlier estimates of this year's claims proved too high. Without that benefit the ratio would have been higher; the release does not give an adjusted figure, and we do not calculate one. The operating cost ratio fell to 12.8% from 13.5%. Chief financial officer Wayne DeVeydt said, according to the news report, that medical costs 'remain elevated relative to historical norms', so the improvement does not mean costs have gone back to where they were before the pressures of recent years. Chief executive Stephen Hemsley said the results 'reflect the discipline and execution our teams are bringing to every part of our business'.
The outlook was raised. The release says full-year 2026 adjusted diluted earnings per share are now expected to be at least $20.50, and GAAP diluted earnings per share at least $19.50, with projected net earnings attributable to shareholders of more than $17.65 billion on a GAAP basis and more than $18.59 billion adjusted. Coin Alert News reports that the previous adjusted guidance was $19.50 to $20.00 and that the new figure is above an analyst consensus of $19.86. The release does not provide an outlook for full-year revenue or the medical care ratio.
The business is shrinking in membership. UnitedHealthcare, the insurance arm, had revenue of $86.3 billion, against $87.1 billion, and earnings of $2.8 billion, against $1.8 billion, an operating margin of 3.3% against 2.1%. It served 48.3 million people, down from 50.1 million a year earlier. By line, the release lists 29.8 million in commercial plans, 7.6 million in Medicare Advantage, 6.7 million in Medicaid and 4.2 million in Medicare Supplement. The news report says membership fell across employer, Medicaid and Medicare Advantage lines and that 935,000 members have left Medicare Advantage since the end of 2025. We have not verified that last figure against the release. The release does not explain the declines, but insurers commonly reduce membership by withdrawing from unprofitable markets or raising prices, and we have no evidence in these sources of the reasons in this case.
Optum, the health services arm, had revenue of $66.9 billion, against $69.2 billion, and earnings of $3.7 billion, against $2.5 billion, a margin of 5.6% against 3.6%. Optum Health, which provides care, had revenue of $22.9 billion, down 10%, but earnings of $735 million, compared with $41 million a year earlier. Optum Insight earned $1.3 billion on revenue of $5.5 billion, up 3%. Optum Rx, the pharmacy benefit manager, had revenue of $39.9 billion, against $39.7 billion, and earnings of $1.7 billion; adjusted scripts fell to 393 million from 414 million.
The cash flow and balance sheet data are in the release. Operating cash flow was $5.1 billion, or 1.2 times net income, and $25.1 billion for nine months. The debt-to-capital ratio was 41.7%, up from 41.2% in the second quarter, with a target of about 40% by year-end. Year-to-date share repurchases were $5.0 billion and dividends paid $6.2 billion. The news report adds that the shares first rose and then reversed during the day, trading at $376.61 late in the afternoon, down 0.51%, but we treat this as a snapshot and not a verdict.
What should readers take from it? For investors, the results show that UnitedHealth's profitability has recovered through tighter pricing, cost control and reserve releases, at the expense of a smaller membership base. For employers and individuals, the shrinkage in membership suggests that the company is choosing which business to keep, and people who see their plan changed or withdrawn at renewal should compare alternatives. For policymakers, the continued fall in Medicare Advantage membership at the largest insurer is relevant to the debate about that program's financing. The sources do not tell us what is happening at other insurers. This article is a news summary and is not investment or insurance advice.

Why This Matters
UnitedHealth is the largest US health insurer, so its medical cost trends are a gauge for the whole health system. Better margins at the cost of fewer members show how insurers are repricing and shrinking unprofitable business, which affects employers and Medicare Advantage enrollees.
